In Italy, 'un caffè' means an espresso, full stop. Drinks are small and the coffee leads. If you want milk, a cappuccino in the morning or a macchiato any time of day both fit the tradition beautifully.

Italians usually drink milky coffee like cappuccino early in the day, with something to eat, and switch to espresso or a macchiato after lunch. It's a gentle custom, not a rule, so order what you love, but it's fun to try.
